HOUSING-FOCUSED
CASE MANAGEMENT
At HERE in Jackson County, all of our services are centered around helping our clients obtain and maintain permanent housing. Every community contact, shelter stay, and service provided are part of ongoing case plans designed to ensure clients secure stable housing.

Housing case management services are available throughout our continuum of programming.
STREET & RURAL OUTREACH
Our Street & Rural Outreach Program is designed to locate those living unsheltered in the community and connect them with services. For many experiencing homelessness, their barriers to housing are also their barriers to accessing services. By connecting with these individuals and families where they are staying, we are able to help them meet their immediate needs such as food, warm clothing, or emergency shelter. We also provide housing-focused case management services including resource navigating and community referrals.
EMERGENCY SHELTER
We provide short-term emergency shelter for households experiencing literal homelessness. Shelter participants are provided with a warm place to stay, an evening meal, and housing-focused case management services.

Our "Code Purple" shelter program provides shelter on freezing nights projected to be 32 degrees or below by the National Weather Forecast.
Rapid Rehousing is a housing assistance program to help those experiencing literal homelessness access permanent housing, ending their homelessness as quickly as possible. Qualifying households can receive assistance with move-in expenses such as security & utility deposits as well as on-going rental assistance. Once housed, clients and their landlords can receive continued support to ensure placements are a success.
